Job DescriptionOverview
IU Health is dedicated to providing comprehensive, patient-centered care for adults with congenital heart disease. We are committed to improving health outcomes through innovative care coordination, patient education, and multidisciplinary collaboration.
Position Overview:
We are seeking a compassionate and organized ACHD Registered Nurse Navigator to serve as a key point of contact for adults with congenital heart disease. The ideal candidate will coordinate care, support transition from pediatric to adult services, educate patients, and advocate for their needs across multiple healthcare settings.
Key Responsibilities:
- Patient Assessment & Monitoring: Check in with patients during acute illnesses, follow up on symptoms, manage medication refills/titration, triage calls, and review test results.
- Care Planning & Management: Serve as a liaison for patients and families, coordinate routine and emergency care, track referrals, and develop individualized care plans with a multidisciplinary team.
- Transition Care: Facilitate the shift from pediatric to adult care, assist with documentation, provide health passports, and ensure continuity of care.
- Patient Education & Support: Educate patients on CHD, treatments, medications, and lifestyle modifications. Connect patients with support groups and community resources.
- Care Coordination & Advocacy: Collaborate with cardiologists, surgeons, primary care providers, and other specialists. Assist with insurance issues, financial barriers, and coordinate care across multiple institutions.
- Quality Improvement & Community Engagement: Participate in initiatives to enhance patient outcomes, collect data, and stay current with advancements in CHD care.
Qualifications:
- Requires an Associates of Nursing (ASN).
- Bachelors of Nursing (BSN) preferred.
- Requires an active Registered Nurse (RN) license in the state of Indiana or an active Nurse Licensure Compact (NLC) RN license.
- Requires that the RN has graduated from a nationally accredited nursing program. 3-5 years of experience preferred.
- Requires Basic Life Support certification through the AHA.
- Other advanced life support certifications may be required per unit/department specialty according to patient care policies.
- Specialty certifications may be required per unit/department specialty in accordance with the department provision of care.
- Requires knowledge of personal computers using various types of word processing and database software.
- Requires clinical work experience, medical terminology.
- Requires understanding of medical record requirements, regulations and policies.
