Description
JOB RESPONSIBILLITIES:
Conducts self ethically in accordance with all relevant standards and regulations and demonstrates respect for patient rights at all times.
Works with an interdisciplinary team to develop and implement a treatment plan of care for patients based on a thorough assessment of the patient's symptom presentation, professional/referral input and all other available data.
Participates as an active member of the Referral Development Team. Appropriately participates in referral development processes and activities as directed.
Assesses patient progress and communicates results and trends to supervisor and treatment team.
Conducts group and individual and family therapy sessions in accordance with patient's diagnosis and needs and in accordance with state/regulatory standards.
Conducts activity therapy sessions and educational group sessions as directed.
Provides ongoing education to patients and families.
Provides appropriate resources and support for patients before, during, and after treatment.
Engages in timely, efficient discharge planning. Works in close conjunction with patient family, nursing staff and attending physician. Displays thorough knowledge of referral/placement process and available community resources and makes effective contact with disposition sources.
Completes progress notes and assessments according to program procedures.
Participates as an active member of interdisciplinary treatment teams.
Submits timely and accurate CQI+ outcome data on a weekly basis. Identifies areas needing improvement and utilizes JMH's performance improvement process.
Provides feedback to supervisor on an ongoing basis regarding concerns, improvements, changes, etc.
Attends Unit and JMH meetings as directed.
Actively participates in department processes as requested.
Monitors Unit safety and reports deficiencies to Director of Behavioral Health Services.
Maintains confidentiality of all information encountered.
Assists with ambulating and lifting patients as needed.
Completes and maintains all required state/regulatory and JMH competencies and training expectations.
Provides clinical supervision to Behavioral Therapist I staff.
Performs other related duties as assigned.

EDUCATION, EXPERIENCE AND TRAINING: Master's degree in social work. Current Indiana licensure as a Licensed Clinical Social Worker (LCSW) required. Minimum of three years of work experience in a similar position required.

SCHEDULE:
8 hours a day, 5 days a week
